Liposuction can be accomplished either with the use of general anesthesia, or with heavy IV sedation, or totally by local anesthesia. Laser assisted Liposuction (LAL) Is the delivery of laser energy to subcutaneous tissue by an optic fiber before or after liposuction Laser Lipolysis is the delivery of laser energy to subcutaneous tissue by an optic fiber without liposuction Laser Lipolysis procedure involves an optical fiber extending 2 mm beyond the tip of a 1 mm diameter microcannula inserted through a tiny fat skin incision into subcutaneous fat. The optic fiber is moved throughout the fat in a pattern resembling the spokes of a fan. Tumescent Liposuction Tumescent liposuction refers to a technique that uses large volumes of very dilute local anesthesia that is injected into fat causing the areas to become tumescent or swollen or firm. Local g anesthesia is widely regarded as the safest form of anesthesia. Because local anesthesia persist for many hours there is no need surgery for narcotic pain medications after surgery. Tumescent technique is safest The tumescent technique for liposuction is unquestionably the f f f safest form of liposuction. There have been no reported deaths associated with tumescent liposuction totally by local anesthesia. Even when general anesthesia is combined with the tumescent technique, liposuction is quite safe provided the volume of fat removed and the number of q p areas treated during a single surgery is not excessive. The dilute epinephrine contained in the tumescent anesthetic solution profoundly shrinks capillaries and thus virtually eliminates surgical blood loss. Mechanism • QuadroStar 980 melts unwanted fat efficiently by selectively targeting adipose fat • Quadrostar 980 heats the water in the dermal collagen to cause shrinkage and ultimately cellular re-growth prompting reduction of lines and wrinkles and p p g providing skin tightening Indications • treatment of fatty tissue areas with the target of fat reduction/removal (abdomen, hips etc) • treatment of areas where liposuction is not appropriate (neck, chin, knee-cap, under bra – line etc.) • treatments of areas resistant to liposuction or with poor response to liposuction p • treatment of areas with previous liposuction and persisting skin laxity • contouring areas with deformities from previous liposuction QuadroStar 980 specificationsthe first table top, high power cw mode laser for – lipolysis • 25 Watt on tissue • 980nm • 200, 300, 400 and 600 µm fiber adaptable • cw, varix, repeat, burst and single mode • upgradeable with second wavlength (532 nm) • sterilized fiber sets for lipolysis (300 and 600 m) • 19 kg - portable g p Surgery procedures • QuadroStar+ • preparation accessories • preparation tumescent solution • preparation lipolysis set • lipolysis surgery Post-OP care • Picture records • Massage Pre-OP preparation• Equipment preparation • Equipment sterilization • Body examination • Pre-OP medication • Surgery line • Picture records • Body disinfection • Anesthesia (tumescent therapy) Facilities and tools • operations theatre (sterile area) • Asclepion lipolyse barefiber set L ( 300 m and 600 µ m fibers, handpieces, cannulas µ , p , Scalpel No. 11, mosquito clips, 4x4 gauze, 20 cc syringe, 5 cc syringe 20 G needle needle pot marker bandage spinal needle, 25 G needle, kidney pot, skin marker, bandage, drainage set, suction system Fiber set L • 2 handpieces (300 µm and 600 µm) • 1 fiber 300 µm/3m and 1 fiber 600 µm/3m • 1 x 20 cm and 1 x 8 cm cannula for 600 µm, 1 x 25 cm and 1 x 20 cm and 1 x 8 cm cannula for 300 µm ![]() Fiber set L • 2 handpieces (300 µm and 600 µm) ![]() medications Midazolam (Dormicum ™, Benzodiazephrine) (oral) Tumescent solution (injection) • 0,9% normal saline 1000 ml • 2% lidocaine 50 ml • Adrenaline 1: 100.000 1 ml • 8,4 % sodium bicarbonate 12,5 ml • 10 mg triamcinolone acetonide 1 ml • total concentration 50/1064.5 = 0,093 % lidocaine • tumescent injection volume is 150 – 250 ml cc per 10 x 10 cm area j • at incision points more volume is required • Less volume is required if liposuction is not performed • volume decision to be taken by physician • prepare tumescent as an anesthetic • Use tumescent cannula or spinal needle with 20 ml syringe or pump to inject solution into fat layer • Spread tumescent evenly in a fan shape ![]() Local anaesthesia injection • use fan method according diagram below ![]() ![]() Body examination and marking ![]() preparation of fiber - set L • insert fiber into handpiece p • fiber has to jut out of cannula 5 - 7 mm in order to prevent heating of cannula and thus skin burns, fiber must be fixed ![]() Surgery preparation • QuadroStar 980 : check function and select correct cw – mode • connection of fibers • safety goggles • red aiming beam must be visible (brightness adjustment) Parameter setting • tight tissue; cw, 6 - 10 Watt, 300 µm fiber • floppy tissue: cw, 4 - 8 Watt, 300 µm fiber Experienced user may run high effective and high speed procedures by using 600 µm fiber, cw, 15 – 18 Watt ![]() • quick forward and backward move cannula creates channels and septum in fat tissue • Speed around 10 cm per second ![]() Clinical results Lipolysis Using a 980-nm Diode Laser: A Retrospective Analysis of 534 Procedures Jean Pascal Reynaud, Martine Skibinski, BenjaminWassmer, Philippe Rochon and Serge Mordon ![]() ![]() Patient satisfaction (laser lypolysis) Lipolysis Using a 980-nm Diode Laser: A Retrospective Analysis of 534 Procedures Jean Pascal Reynaud, Martine Skibinski, BenjaminWassmer, Philippe Rochon and Serge Mordon ![]() Time table respective power applied QuadroStar power available: 25 Watt in tissue ![]() QuadroStar 980 advantages • volume reduction and skin tighteningsmoother • skin with less deformities compared to liposuction • faster healing with smaller incisions, less bruising, reduced pain minimal blood loss and swelling • cw – technology allows superior controlling of thermal effect vs. competing high peak power pulsed laser • competing system using short (200 µs) peak power pulses cause extreme temperatures in the tissue.
